If a provider/supplier, or any owner, managing employee, authorized or delegated official, medical director, supervising physician or “other healthcare personnel” if excluded or debarred from Medicare, Medicaid, or any other Federal health benefit program,
OR
If any of those people or entities are debarred, suspended, or otherwise excluded from participating in any other Federal procurement or nonprocurement program or activity.

Screen monthly and keep records – OIG/LEIE and GSA lists as well as state lists for Medicaid
Screen BEFORE you acquire a health care business
If you identify an excluded, debarred or suspended person – take swift action to address it
